The medical questionnaire is a short set of health questions linked to the treatment you choose. It gives a licensed doctor the information they need to decide whether a treatment is safe and suitable for you, without an in person visit.
How it works
- You answer questions about your health, history and any medicines you take.
- A doctor reviews your answers, usually within 24 hours.
- They may follow up with extra questions if needed.
- If suitable, a prescription is issued, otherwise you are told why.
Answer fully and honestly
Your answers guide a medical decision, so being complete and truthful keeps you safe. False or misleading information can make a treatment unsafe, and it affects whether a refund applies.
